• Earn as high a cumulative GPA as possible and maintain at least a B in every class -

  • The higher the GPA, the better chance to compete for scholarships and admissions into the college of my choice.

  • 3.75 GPA recommended and 2.5 minimum GPA required throughout high school.

• Pass the State English, Math, & Science standardized assessments.

• Stay in school & on time with my graduating class from year to year and graduate with a regular diploma with my class, the class with whom I entered high school.

• Take rigorous, challenging courses throughout high school all through to graduation.

  • Rigorous includes accelerated courses, AP, Running Start, College in the High School courses. In addition, students are required to successfully complete, at a minimum, the following courses:

    • Four (4) years of English

    • Three (3) years of mathematics:

      • Algebra I plus two higher-level classes such as Algebra II, Geometry, etc

    • Three (3) years of Science:

      • Must include one year each of at least two of the following courses: Biology, Chemistry, and/or Physics

    • Three (3) years of Social Studies

    • Two (2) years of language other than English (must be the same language)

  • Running Start classes count: 1 college quarter = 1 high school year

• Enter college in the fall immediately following high school graduation and start at college level ELA & Math.

• Earn an Associate’s or Bachelor’s degree within 6 years of high school graduation.

I understand the above academic goals, agree to make them my own, do my best academically each school year, attend school regularly, complete and turn in all homework, study for quizzes and tests, actively participate in my high school and Upward Bound classes, have regular, direct, respectful communication with the Upward Bound staff and my teachers, and represent myself, my high school, the university, and the Upward Bound program in an upstanding way.

I understand if I do not uphold this commitment, I will be placed on UB academic probation, required to attend assigned study sessions and risk my participation in the Upward Bound program, as it is a privilege for select students who want to go to college and are taking action to get there and be successful.

TRIO Upward Bound strives to provide a safe and healthy environment in which student's attitude and behavior are essential and every individual makes a difference in the quality of the experience. To create a community atmosphere, students agree to abide by these guidelines of behavior while participating in Upward Bound:

• In the local community and beyond, to all areas we visit, will show respect at all times to people and for property, including others’ personal possessions, privacy and feelings. 
• Do not use your cell phone during programming unless allowed by TRIO UB staff.
• Do not use rude or obscene language or gestures or make reference to violent offensive actions.
• Dress according to school policy or as asked by TRIO UB staff.
• Do not participate in any activity that might endanger myself, another student or staff.
• Follow established curfew hours and follow safety directions provided by UB staff and personnel in the places we visit.
• Stay with WSU Upward Bound staff at all times and follow agenda times promptly. 

If the student is unable to meet these expectations of behavior:

• First Offense: verbal warning. 
• If the behavior persists, parents will be notified by phone and requested to help their student make more positive choices. 
• If the student’s behavior does not improve, the student will be asked, at the discretion of the program, to leave. Parents are responsible for the student’s travel home from wherever we are when the student is asked to leave.

The following behaviors will result in immediate expulsion from the program and potential criminal conduct will be reported to the police:

• Any crime, including the use, possession or distribution of firearms, alcohol, drugs, tobacco, or any illegal product. If student possesses prescription or over-the-counter drugs, they should be included in the medical form or it will be considered in violation of the code of conduct.
• Discrimination or harassment, as defined in the WSU Policy Prohibiting Discrimination and Harassment, EP15 (opens in new tab).
• Bullying or hazing or other behaviors that endanger yourself or others, which are disruptive to other participants or guests, or which cause damage to facilities in use.
• Aggression, physical or verbal abuse of any kind including, but not limited to, hitting, kicking, biting or pushing another student or staff member.
• Engaging in sexual contact or conduct with others.
• Entering portions of facilities or utilizing certain equipment which the program identified as restricted.
• Failure to follow staff instructions resulting in situations that put themselves, other students, or employees in physical danger.
• Leave an activity or workshop without the permission of the activity’s supervisor or area staff member.
• Students threaten to harm themselves or other students or actions reasonably likely that result in harm to themselves or others.

I, the parent or guardian, hereby grant permission to Washington State University (WSU) TRIO Upward Bound - Columbia Basin Program, to photograph, take videos of my child or otherwise have images, voice recordings, videos made (including but not limited to: photographs, digital photographs, video or digital moving images, and/or voice recordings) for WSU TRIO UB publication or promotional purposes in any medium (including but not limited to: print media, newspaper, television, video, motion picture, website on the internet, social media, etc).

I additionally consent to the use of my child's name and/or interview comments in connection with WSU TRIO UB publication or promotional purposes in print media, newspaper, television, video, motion picture, or website on the internet.

I understand that consent to use my child's photos/videos is not a condition of participating in the program or program activities and that consent can be refused without any impact in the ability to fully participate in the program.

We understand that we can revoke this consent at any time upon written notice to WSU TRIO UB, at which times either or both of us will sign a copy of the denial (below) for use of images or voice recordings.

To receive services from this TRIO Upward Bound program, we require full access to all student records. 

Please carefully read the disclosure statements below. 

I, therefore, authorize all of my student records, data, information, and other related documents, to be disclosed to, and by, the Washington State University Tri-Cities Columbia Basin TRIO Upward Bound program for the purpose of, but not limited to: program recruitment and planning, student services such as assisting with achieving educational and financial aid goals, collecting program statistics and performance reporting. 

These records may include:

• Secondary and Post-Secondary Educational Records
• Transcripts/Grade Reports/Evaluations
• Post-secondary Education Status, progress and degree completion
• Income verification information
• FAFSA completion & related information
• Department of Education requested information
• Test scores and reports

I further authorize this information and the nature of my program participation to be shared with and by entities such as university personnel, the National Student Clearinghouse, state longitudinal database systems, educational and community partners.

I have carefully read this record disclosure consent and fully understand it's contents. I am aware this document is a contract. I sign it freely and voluntarily. I also realize this document will be considered effective from the date signed unless rescinded in writing.

I/We certify that all information provided on this TRIO Upward Bound (hereafter referred to as “UB”) application is true and correct to the best of my/our knowledge. 

I/We understand the completion of this application does not guarantee acceptance into the TRIO UB program. 

I/We understand the information provided on this application will be held in confidence by the TRIO UB program according to the Federal Privacy Act, Family Educational Rights and Privacy Act (FERPA), and as allowed for by the UB Record Disclosure consent form on file. 

I/We understand signature below indicates UB participant will follow the UB code of conduct, UB academic agreement, current school rules and regulations in place at my local school as stated in the student handbook as well as the rules and regulations of the colleges, universities & places we visit (strictly enforced).

I/We authorize TRIO UB staff to communicate with teachers, counselors, school staff and school and other administrators in an effort to obtain and exchange information pertinent to providing recruitment into the program and TRIO UB services.

I/We authorize the release and exchange of student financial aid information, post-secondary enrollment information, and transcripts from, but not limited to, colleges, state and federal government agencies to the TRIO UB program.

I/We grant permission for my child to attend TRIO UB field trips.

I/We authorize TRIO UB to seek medical attention for UB participant if needed. 

I/We understand that if accommodation for a disability is needed to participate in UB, or any of its scheduled activities, I must contact Upward Bound at (509) 793-7678 with ample notice prior to the activity. 

I/We release TRIO Upward Bound and WSU of any and all liability. 

I/We understand that TRIO UB, WSU, as well as the Washington State TRIO association, Regional TRIO association of which this UB program is a part, and the Council for Opportunities in Education, Moses Lake High School, Vanguard Academy and Warden High School have full and absolute permission and all rights to copyright, publish, display, and use for any legal purpose all photos, videos, and/or voice recordings together with descriptive text or statements, in which I or my child/children and/or property appear from this date forward unless we notify WSU-TC CB TRIO UB in writing.

I understand, if accepted, I will fully participate throughout high school, and until college graduation, including academic year and summer activities. I also understand that if I do not, I may be dismissed from the program.
