Before filling out this application you will need the following information:

♦ GRADES & TEST SCORES

Grades – most recent transcript or grade report with cumulative GPA signed and dated by school counselor if it’s not a transcript.
- You can upload your transcript or grades directly into your application, or email them to trioub@mlsd161.org or drop them off in our office at Moses Lake High School in the library.
- Moses Lake High School students send your transcript to trioub@mlsd161.og via https://www.parchment.com/u/registration/25184/account

♦ RECOMMENDATIONS/REFERENCES

Please provide two recommendation forms from people who can tell us about you. Click on Recommendation Forms to download!
- First, a teacher at your school who knows you, has had you in class, and can vouch for your grades, skills, talents, and work ethic.
- Second, from either a teacher, counselor, coach, youth worker, church official, etc. who knows you, who can speak on behalf of your talents, work ethic, dedication, commitment and your potential.
- You can upload these directly into your application, or email them to trioub@mlsd161.org or drop them off in our office at Moses Lake High School in the library.

♦ WRITING SAMPLE

Please type and attach a writing sample explaining who you are (autobiography).
Make sure to use the following header:

Your Name, School, Grade Level

This is a very important part of the application, so be thoughtful and detailed.
